By submitting this form you are requesting to use the 1 fine waiver class allowed per
member, within a 12 month period. You MUST attend a scheduled educational class
virtually within 30 days of the fine issue date.
*If request form is not returned and class not attended, fine will not be waived and
fine amount will be due.
